Starting your HGV journey begins with completing your D2 and D4 forms. These forms are required by the DVLA before you can begin practical HGV training and gain your provisional HGV entitlement. What Are the D2 and D4 Forms?

The D2 and D4 forms are official DVLA documents required when applying for your HGV provisional licence.

  • D4 Form – This is the medical examination form that must be completed by a qualified doctor to confirm you meet the medical standards required to drive an HGV professionally.

Both forms must be completed and sent together to the DVLA.


What Is the D2 Licence Application Form?

The D2 form is the application form you complete to request provisional entitlement for the HGV category you wish to train for. This is similar to applying for your original car licence, but specifically for commercial vehicle categories.

These specialist services are often significantly cheaper than booking through your own GP surgery while still completing exactly the same DVLA-approved medical examination.


Where Do I Get the D2 and D4 Forms?

You can usually collect D2 and D4 forms from larger Post Office branches. However, if your local branch does not have them available, we can help.
